      No one knows who first used it or created the word. This is the best I could find on wiki-pedi. You can go there yourself and read up on it.
      "Its first known use as a verb meaning to have sexual intercourse is in "Flen flyys", written around 1475."

        The word was not offensive back then. It was just a word as commonly used as the word sex itself and no one calls the word sex offensive ;)
