Looks pretty simple to me. She unhooked the heavy cape, it was supposed to release and didn't. Consequently, when she went to continue up that last step, she was pulled backwards. I'm not a Madonna fan, but really, accidents do happen, even to people who are sober! Give it a rest already!

The cape she was wearing was suppose to come off when the girl tugged at it, unfortuneately for Madonna it didn't, so down came cape, Madonna and all. :)

I never heard that she was an alchky. She said that something caught her long hem. Not everyone that falls is a drunk, Hector. Lots of models have fallen coming down the runway, b/c their shoes are so unstable.
