    Take the tooth out. Please note, I am not a dentist nor am I studying to become one like you are. Refer to your text books.

    Assuming you are using either the ISO or the Palmer Notation, the tooth is the lower left second molar. As it erupts at age 11/12. it has decayed rapidly. The treatment depends upon several factors including dietary reform and the presence of the third molars. Colleen suggests extraction which is probably the way forward but why not post on a dental site? www.GDPUK.com is a good one.
