It means keeping yourself clean. Keeping your hair combed. Wearing clean, unwrinkly clothes. Wear clean, nicely polished shoes......if they're leather, rubber, or plasic. If they're canvas, keep them clean. For women, - - - -at least wear some light colored lip stick, if not powder .. Every woman looks better with lip stick on.

Different things.  If you look nice, perhaps you are dressed well, as MCM suggested. Personal grooming helps, as billyboy1 included.  
I like what malinip1 and Rebbe Jon said.  A pleasant smile and just something about your inner goodness that reflects outwardly....those are "nice". 

Nice, in dictionary terms, in the context to which you are referring, means having an appearance that is pleasing, agreeable, delightful, and/or pleasant.

Looking nice means (to me) well turned out! well groomed, tidy hair, shiny buttons, nicely dressed and dont forget the lippy! (lipstick) and well manicured nails.  Looking nice to some people could mean you look drop dead gorgeous...........attractive, pretty. (A bit like me really ha!)
