Go online to socialsecurity.gov/applyfordisability/.

Gather your tax forms, your medical records and, if you have one, your form DD 214, which includes your military service discharge information for any time you were on active duty.

Click on Step 2 "Fill out the online Disability Benefit Application" at the bottom of the webpage. This will open a new window that will require you to put in your Social Security number and say that you're the person applying for the disability. Continue answering questions about your personal history, residential history and military service.

Click on Step 3 "Fill out the online Adult Disability Report." Select "I have read the privacy act statement" then choose "Start Report." You'll have to answer specific questions about your work and medical history, so it helps to have your resume and medical bills and reports available.

Open form SSA-827 by choosing Step 5 on the original webpage. Print the form and sign it. You can either mail it or take it in person to your North Carolina Social Security office. You can locate the office by zip code at the Social Security Office Locator, linked in Resources.

Wait to be contacted by a social worker who will let you know whether your claim has been accepted or denied. If it was denied, file an appeal through the Social Security office. It if gets denied a third time, contact a disability lawyer to help you win your case.
