Sounds like she may have worms (at the least) or may have some sort of other medical issue. Have a vet check her.

Her first visit she weighed a little over 5 lbs & he said she had tartar on her teeth & would need to gain weight b/4 he would put her under to clean them. She was on meds for a while too from the 1st visit. Didn't help. On her last visit she had lost weight & my vet treated her for tape worms along with 1 of my other cats. I just don't know what to do. She's active but just seems to be melting away.

If she only got one treatment of wormer, then she may still have worms. The first treatment kills the adult worms, then the eggs hatch so she would need a second treatment. Since you say she is eating normally, her teeth can't be bothering her too much or she may just be dealing with the pain. You need to have the vet look at her again or find another vet that can help her. She may need some blood work done to make sure there is no other issue.
