    I-485 Application Form


    The I-485 application"" form has five parts to be filled out by an applicant for a green card. The first part requests information about the applicant such as name, address, date of birth and current status. The second part requires the applicant to indicate the reason for the application for the green card. The third part requests processing information such as place of birth, current occupation, immediate family information, past crimes committed and memberships in organizations. The fourth part applies only to those who are petitioning for an accommodation due to an impairment or disability. The fifth and sixth parts of the application form require an applicant's signature and confirmation that all questions were understood and are truthful.


    Form G-325 A


    Form G-325A is a biographic information sheet required for all green card applicants between the age of 14 and 79. It asks for an applicant's family information, residence for the past five years, employment history and current occupation.


    Form I-693


    Form I-693 is a medical examination form that is completed by a designated physician, signed, sealed and then given to the applicant to include in the green card application package. The designated physician will charge a fee for the examination. This form cannot be opened by the applicant once it's sealed.


    Form I-864


    Form I-864 is required to demonstrate that an applicant has sufficient financial resources to live without becoming reliant on government welfare. The form can be completed by the petitioner or a sponsor and asks for general biographic information, employment and income, assets"" and tax information.


    Passport, Birth Certificate and Fees


    A green card application requires the applicant to submit a copy of a valid, current passport of his nation of citizenship and a birth certificate, notarized birth document or other official record of birth. In addition, an applicant must submit a filing fee for the I-485, which varies between $985 and $1,070 as of 2011.
