Check with the IRS or your broker. My sense it's possible if you've otherwise met the requirements for the Roth IRA withdrawal. My guess is such a withdrawal and paperwork must be done in the same calendar year, hence, if you made a withdrawal near the end of December, you may be out of luck. You see, your broker can change or "amend" the paperwork if caught in time during the year. After the new year, however, the forms get sent out to you and the IRS, thus making any changes or amendments more difficult.
